We provide disability support services to individuals in South West Western Australia. Included in these services is the development and management of a coordinated plan of action which guides the weekly sessions delivered by our support workers, who we call mentors.
We can tailor our services to fit within your NDIS plan and budget. Our coordinator would be happy to meet with you to discuss your individual needs.
Our Services
How it works:
Our coordinator works closely with our clients and their families, therapy teams and mentors (support workers) to develop written plans based around goals to be achieved.
Our mentors (support workers) deliver these plans via regular 1:1 sessions with clients. These sessions can take place in the home, the community or the workplace, depending on our clients’ goals.
We offer 2 tiers of services, dependent on the level of clinician/therapist involvement required*.
Both tiers include regular and frequent communication between clients and our Mentor Matters WA coordinator.

Tier 1
Tier 1 ensures that therapy goals are carefully woven into the delivery of supports. In this tier, we work with clinicians and our client to develop a detailed written support plan, stating SMART goals and strategies, based around our client’s needs and therapy goals.
This plan is delivered by a mentor (support worker) and is monitored, reviewed and evaluated by the team in 6 monthly planning cycles.
Tier 2
Tier 2 varies from Tier 1 in that our client’s clinicians are not involved in the planning process. Our Mentor Matters WA coordinator will develop a written support plan based around our client’s/family’s needs for the mentor (support worker) to follow.
This is reviewed 6 monthly via a meeting with client, coordinator and mentor (support worker).
